With Wise Choice, a team of dedicated care professionals work with you to create a personalized care plan that identifies your goals, priorities, and options for aging well. In this fee-for-service program, we work closely with you to provide the information and options to make informed choices about the best ways to age wisely and to support your unique lifestyle and needs-whether it is at home or elsewhere.​

Personalized Support
With Wise Choice, a team of dedicated care professionals work with you to create a personalized care plan that identifies your goals, priorities, and options for aging well. We collaborate with you to provide the information and steps needed to put your plan in place. WISE Choice includes these core services.

• Comprehensive Assessment to identify your goals and needs. This step, which often includes other family members, helps all parties understand your unique circumstances and concerns. It evaluates physical safety, nutrition, cognition, social well-being, medical history, legal issues and financial issues. The assessment Includes recommendations and referrals to support you in the best way possible.
• A Care Plan tailored to your specific needs. Your customized plan provides a roadmap of care developed for you and is updated as goals are met and circumstances change.  
• Support and guidance to help you think through specific decisions.
• Regular check ins, monitoring your progress and identifying concerns as they arise. 
• Assistance navigating and managing services such as transportation, cultural activities, housekeeping, purchasing supplies, home maintenance, personal care assistance and more.
• ​A trusted, local point of contact for out-of-town family members.

Nourish Nutrition Services

• Individualized grocery shopping– Create your list and a JFS team member will do your shopping for you.
• Wise Meals at Home– Nutritious, locally sourced, prepared meals delivered to your home. Two healthy meals/day can make all the difference in the world to staying healthy and promoting well-being.
• Connecting you to dieticians, health coaches, and other nutrition professionals.

​Medical and Health Support

• Medical Accompaniment to medical appointments, tests and surgeries to improve continuity of care. WISE Choice staff can take notes for clients and family members as needed.
• Navigation and coordination of services for chronic/progressive illness, including the facilitation of communication between providers, you and other care partners.
• Durable medical equipment, safety inspections and home modification support.
• Coordination of discharge planning with hospital staff.

Transportation

• Professional, door-through-door transportation services to grocery stores, pharmacies, medical appointments, and community events. JFS offers wheelchair lifts and ADA support services. Caregivers are welcome to ride along.

Social Connections

• Social enrichment, including WISE Senior Center, a congregate meal program that includes concerts, games and trivia, current events and mind-sharpening cognitive offerings.  
• Wise Comfort Line: A weekly call just to chat or identify emerging needs.

Mental Health Support
Direct referrals to JFS therapists, through our Thrive program, who focus on life transitions, grief and loss, coping with medical illness, relationship and family dynamics and more, as well as our extensive network of community therapists and psychiatrists for specific needs and medication evaluation.

​Home Support and Transitions

• Evaluation of the home and coordination of modifications for safety and fall prevention.
• Identification and coordination of home care services now and for the future.
• Continued conversations to discuss maintenance of independence, health care, living arrangements, etc.
• Consultation and information regarding alternative living facilities, touring with client and families to discuss advantages of each.
• Help in downsizing and moving when appropriate.

Insurance, Legal and Financial Matters

• Support to navigate insurance issues, including health, life, long-term care, Medicare, Veteran’s affairs.
• Options for bill paying, checkbook balancing, and consultation with accountants or Power of Attorney representatives on behalf of our clients. 
• Referrals to our network of financial and legal professionals specializing in elder care, elder law, estate planning and management.

End of Life Decisions and Care

• Assistance with understanding and enrolling in hospice care and referrals for end of life arrangements, including religious and culturally-sensitive resources and services at end of life.
