If you are an older adult or caregiver in Washtenaw County and need help, WISE Case Managers can provide assistance in finding the support you need.
Care Coordination and Management
Case Managers can provide older adults 8 weeks of support by navigating programs and services to help you better manage at home. We work with you to understand what your goals are and pathways to accomplish them. Examples of ways we can help you include improving connections to the medical system, finding reliable transportation or nutrition services, as well as ensuring you have the support you need to safely remain independent in the place you choose to live. Every situation is different and no plan is the same.

Services include:
  • Working with you to assess what you need and to understand your goals
  • Developing a plan with you to best meet those goals
  • Providing Care Coordination – regular meetings or discussions with you for up to 8 weeks that include community referral and resource navigation for the services you need
  • Enrolling you in programs within JFS for any critical services you need, such as crisis intervention, nutrition, transportation or counseling
  • Checking in with you for two months following care coordination services to make sure plans are in place to maintain your progress before closing your case.

​Nourish Nutrition Services
Healthy food options catering to specialty diets.
  • The Specialty Food Pantry caters to dietary needs and restrictions and cultural dietary guidelines. Offering curbside pickup or home delivery for homebound older adult
  • Meals on Wheels for eligible homebound older adults
  • Referrals to dieticians, health coaches, caterers and other nutrition professionals

Helping you to get where you need to go-- when you need it.
​
JFS GO is a transportation service that provides a sliding-scale-fee option to individuals and families, so that they can access grocery stores, pharmacies, attend medical appointments, and stay active in the community through events and programs.

WISE Senior Center
Older adults participate in socialization activities including music, art projects and outings, as well educational workshops. Congregate meals are served on Mondays, Wednesdays, and Fridays. Visit the WISE Senior Center page for more information.

New American Club
The New American Club (NAC) for Older Adults is a social and community resource support program for Russian-speaking seniors who have come from the countries of the former Soviet Union. Visit the NAC page for more information.

WISE Comfort Line
Trained volunteers contact older adults to reduce social isolation, identify emerging needs and brighten their days. Older adults can expect weekly calls from their matched volunteer. Volunteers screen for critical needs and if these are identified, the volunteer makes a referral to the WISE team of social work professionals for follow up.  Sign up to volunteer.
